How We Work Together
A Continuum, Not a Competition
TrueNorth values the relationships patients already have with their outpatient providers.
When appropriate and authorized, we collaborate with referral sources to support continuity of care.
Our objective is not to unnecessarily replace established outpatient relationships. Instead, TrueNorth can provide a temporary higher level of structure and stabilization before helping the patient transition to appropriate continuing care.
Level of Care
When to Consider PHP or IOP
A referral may be appropriate when a patient:
- Is deteriorating despite traditional outpatient treatment
- Requires more frequent therapeutic intervention
- Is transitioning from inpatient or residential treatment
- Has experienced repeated crises or functional deterioration
- Needs structured treatment for a substance use disorder
- Requires integrated treatment for co-occurring conditions
- Or would benefit from intensive treatment while remaining in the community
