Depression is not always visible from the outside. Many adults keep showing up for work and for the people who rely on them, while running on far less than they used to have.

It may look like mornings that take much longer to start, a workday that stretches on without moving forward, concentration that slips in conversations you once handled easily, or sleep and appetite that no longer follow their usual pattern.

It can show up as a shorter fuse at home and a slow drift away from plans and friendships. None of that is a diagnosis. But when low mood, exhaustion and loss of interest persist and begin to set the shape of your days, it may be worth asking a clinician what would genuinely help.

When Weekly Therapy May No Longer Be Enough

Weekly therapy helps a great many people. Sometimes one hour a week is not enough to keep pace with what is happening the rest of the week.

Needing more support is not a failure of therapy. It often means the level of care no longer matches the level of difficulty.

A more structured level of care may be worth discussing when:

  • Symptoms have continued or worsened despite consistent outpatient treatment.
  • Work, study or responsibilities at home have become hard to keep up with.
  • You are leaving a hospital stay and still need close clinical support.
  • Medication has recently changed and closer follow-up may help.
  • Depression is occurring alongside anxiety, trauma, or alcohol and substance use.
  • You have withdrawn from most things outside of obligations.

Depression tends to shrink a person's week. Structured programming is designed to work in the other direction.

  • Closer Clinical Review Symptoms, safety and response to treatment can be reviewed more often than weekly.

  • Practical Skills Sessions focus on tools you can use between visits, not insight alone.

Inside the Program

What Treatment May Include

Depending on individual clinical needs, treatment may draw on several approaches at once.

Group therapy is typically a central part of both programs. Sessions may cover understanding depression, working with difficult thoughts, rebuilding routine and activity, managing stress, and tending to the parts of life that have narrowed.

Individual sessions give you a private place to work on what is specific to you and to review how treatment is going.

Psychiatric evaluation and medication management are available when clinically appropriate and coordinated with the rest of your plan. Any decision about medication is made with a prescriber, based on your history and your preferences.

Family involvement may be part of treatment when you want it and when it is clinically appropriate.

Levels of Care

How PHP and IOP May Differ for Someone With Depression

Both programs are outpatient. The difference is how much structure each one provides.

Partial Hospitalization Program (PHP)

PHP is the more structured of the two, with programming typically several days per week and more hours in the day.

It may be appropriate when symptoms are significant, when daily functioning has slipped, or when someone is stepping down from a higher level of care.

Intensive Outpatient Program (IOP)

IOP provides structure at a lower intensity, with fewer hours each week. Day and evening options may be available.

It may suit someone holding a job or caring for a family, or someone stepping down from PHP who still benefits from regular groups.

Many people move between the two. You may begin in PHP and step down to IOP as symptoms settle, or begin in IOP and step up if more support is needed. In both, you return home at the end of the treatment day.

Co-Occurring Concerns

When Depression and Substance Use Occur Together

Mental health and substance use conditions frequently occur together, and treating one while setting the other aside often leaves the problem in place.

Alcohol or other substances may begin as a way to sleep, to quiet anxious thinking, or to feel briefly better. Over time that relief can become its own difficulty, and it can make depression harder to treat.

Continuing Care

What Happens After the Program

Structured treatment is a chapter rather than the whole story, and planning for what follows begins early.

As treatment progresses, your team works with you on what continued support may look like. For many people that includes ongoing individual therapy, psychiatric follow-up when appropriate, and a practical plan for noticing early warning signs sooner.

A step down from PHP to IOP is often part of that path, so the change is gradual rather than abrupt. If you already have a therapist or prescriber, we can coordinate with them when appropriate and authorized.
